About Long Term Care
  • Long term care is the care of an Elder or individual with a disability who requires on-going assistance with daily living activities such as bathing, dressing, eating, shopping, and cooking.
  • Long term care supports Elders and their families with medical, personal, and social services delivered in a variety of settings, ranging from a person's own home to institutional settings, to ensure quality of life, maximum independence, and dignity.
  • In American Indian and Alaska Native communities, long term care also involves maintaining cultural values in the delivery system.

  • To qualify for Early Bird registration, please send the registration form and payment postmarked on or before June 30, 2007, by regular US Postal Service.
  • Regular registration rates will apply to those registrations postmarked between July 1, 2007, through July 31, 2007.
  • Please do NOT mail or fax registrations after July 31, 2007.
  • All registrations after July 31, 2007, will be processed on-site at the daily rate of $65.00.

To submit an abstract focusing on the best practices of your program, please respond to the questions below and limit your response to approximately 400 words. Be sure to be as specific as possible. Submit abstract to ltc@nicoa.org or fax to (505) 292-1922 on or before May 21, 2007.

  • What is the population served by your program? Please indicate rural or urban.
  • What are the specific services offered by your program?
  • How did you identify the need for these services?
  • What is the length of time the program/services have been offered?
  • How are the program/services financially supported?
  • How do you measure the success of your program?

Also, please include your name, title, organization, address, telephone number, fax, email, and a brief biography with your abstract submission on or before the deadline of April 30, 2007. If you are selected, you will be notified before May 31, 2007, so tht you can register at the early bird conference rate.

CANCELLATION: Post Conference Intensives have been cancelled!
To expand your enjoyment and quest for knowledge, we are offering two post conference intensives this year on Friday, September 7, 2007. The intensives will prove an opportunity for you to learn more about two models of long term care that have been highlighted at previous conferences: the GREEN HOUSE® and the Program for All-inclusive Care for the Elderly (PACE). Both sessions are scheduled for 8 a.m. - 5 p.m. with lunch included for a cost of $100. This is a great opportunity to learn more about these exciting projects.

  • GREEN HOUSE® is a social model of care combined with direct care staff empowerment to foster an atmosphere of greater autonomy, privacy and choice for residents. It is a shift from the institutional, medical model of care to an actual home environment. The model features a series of 10-unit homes with a staffing plan that elevates the responsibility and position of the direct care provider, and utilizes a clinical support team from a larger institution who visit the home at regular daily or weekly intervals, providing nursing, physical and occupational therapy and other clinical services.   • PACE is a planned approach to chronic care serving individuals 55 and over who meet nursing facility level of care. PACE prolongs well living in the community through health promotion/disease prevention, support for family/caregivers, social opportunities and personal care and housekeeping services. PACE is a managed care program that includes all needed medical and supportive services: services are reimbursable through Medicaid and Medicare.
